Insurance Information
Envision is a proud in-network provider for many commercial insurance plans including: Aetna, Blue Cross Blue Shield, CIGNA, Meritain, United Health Care, and affiliated networks, as well as several Employee Assistance Programs (EAP). For any out-of-network plans, we will gladly submit to your health insurance company on your behalf or provide you with a Superbill.
Services may be covered in full or in-part by your health insurance or employee benefit plan depending upon your benefit coverage. Please check your coverage carefully by asking the following questions:

Cancellation Policy
A cancelled or missed appointment impacts our ability to best serve our patients. Kindly provide 24 hours notice of your request to reschedule or cancel. If you do not show up for your scheduled appointment, and you have not notified us at least 24-hours in advance, a fee of $50 will be charged.
Payments
Cash, check, and all major credit cards are accepted for payment. Health Savings Account (HSA) cards are accepted for allowable services.
